For Medicare Advantage HMO patients only – this visit is covered under your insurance plan.

During your Annual Health Assessment, you will meet with a Nurse Practitioner who is part of your care team and works closely with your primary care physician.

Your visit will include a review of your new and current health conditions, assessment of prescriptions, discussion about recommended screenings, advice tailored to your personalized health plan and more. Together, we will develop an individualized care plan and road map to help you stay healthy.

Patients may only complete one AHA per calendar year
